No Show Policy
Our clinic has a strict no show policy. Appointments that are not rescheduled 24H in advance are subject to fees for re-booking and procedures not rescheduled 5 days in advance are subject to fees for re-booking
Post Surgery follow-ups
Patients for routine day surgeries (hernia, laparoscopic cholecystectomy) have the option to follow-up with Dr. Sample, but are not required if they are feeling well with no concerns. There is also the option of a telephone follow-up that can be booked through the clinic.
IUD
Dr. Ochs provides expert service in IUD insertion. The following information can help you prepare for the procedure.

If you are scheduled for a surgical procedure with Dr. Sample, the following information should be followed carefully for procedures performed in Covenant and AHS facilities such as the Grey Nuns.
Grey Nuns Hospital
Dr. Ochs, Dr. Kleiman and Dr. Sample are members of the medical staff at the Grey Nuns Hospital, part of the Covenant Health group. Your surgeries and endoscopy procedures will be at the Grey Nuns Hospital.. If you choose the map option, just enter your current location into the starting point.
